Each session runs on a Tuesday, from 9.30 - 11.30 on the following dates:

About Bridges:
Bridges is a social enterprise focusing on personalised self-management support for people living with long-term conditions. Bridges is based on self-efficacy theory (Bandura) and covers key principles that enable staff to provide personalised self-management support within every healthcare interaction. Bridges courses are co-delivered by people with lived experience.
The Bridges training aims to build practitioners’:
- Knowledge of Bridges core principles of personalised self-management support and Bridges’ underlying evidence base that supports its effectiveness.
- Skills and tools to embed the core principles throughout your clinical practice.
- Confidence in working in this way with patients and in teams.
Workshop content:
- Workshop 1: This workshop introduces the Bridges approach to Self-Management and how it relates to your service. We focus on identifying what most meaningful to patients and introduce the concept of self-efficacy.
- Workshop 2: Continues with self-efficacy, focusing on the power of reflection. It then covers the 7 key self-management skills and why language is so important.
- Workshop 3: The 3rd workshop focuses on goals, hopes, fears and risk. It finishes with a section on individual action planning so you are ready with your next steps for how to begin to embed Bridges into your professional practice.
- Implementation phase: There is then a 3 week implementation phase during which there is a break from workshops and participants try out Bridges in practice and complete a case reflection.
- Workshop 4: The group then returns to discuss and reflect how Bridges has gone, and learn new content on collective efficacy and self- management resources. You’ll also start to develop next steps for implementation and sustainability of Bridges in your service.
Follow up support from Bridges:
Attendees also have the opportunity to become a Bridges champion and avail of ongoing support from the Bridges team, including coaching and networking to ensure changes are embedded and sustained.
